This review list is provided to inform you about this document in question and assist you in its preparation.  The trip permission slip is one more small step to protect volunteers and volunteer activities in your town.  It can be adapted to a variety of purposes to protect the volunteer, at least in part, from liabilities related to unpaid supervision or intervention in local town business.

1. Make sure you get signed copies from all parents.  In particular, make sure you get signed copies from any individuals who you believe may be litigious.
